Discovery

Getting to know the individual and how to support of their needs and goals.

The goal of discovery is to create a comprehensive description of someone that includes strengths, interests, preferences for participation, learning and socialization, description of support needs at home and in community settings.  Discovery includes a statement about the person’s goals and what’s most important to him/her/they/them in their everyday life.

Discovery enables the case manager to build rapport, get to know the person intimately, and build trust and strong communication.

The case manager will interview and observe the client and their parent/circle of support and complete appropriate assessments.

A Comprehensive Discovery Profile will describe the individual’s strengths, levels of support, learning profile, personal preferences, medical and behavioral needs, past challenges & concerns, and how they were managed or overcome.

Discovery will include detailed recommendations to enhance community inclusion/participation and overall quality of life.

Current and future goals will be recommended. For example, what does the person want life to look like in 6 months, 1 year, 3 to 5 years.

Assessments may include: Vineland Adaptive Behavior Scales, Support Intensity Scale and/or functional behavior assessment, Personal Outcome Measures

Intake & agreements

The individual and their family will provide the following: 

  • Most recent psychological evaluation & service plan
  • Letter of Intent 
  • Release of information for all service & medical providers
  • Completed interest survey
  • Most recent positive behavior support plan or functional behavior assessment
  • Financial agreement for services
  • Weekly meeting time agreed upon with individual & parent/designated support person

(Discovery completed in 45 days. The case manager may extend Discovery as needed and update the team with a revised completion date.)

Meaningful Life Goals

Collaborating to create relevant and meaningful life goals.

The case manager will collaborate with the person and their circle of support to create relevant and meaningful goals in the following areas:

  • advocacy & life skills
  • healthy relationships – friends, family, day program (including nonpaid relationships)
  • work/vocational goals
  • volunteer/community service
  • plan social activites
  • health & wellness
  • daily & weekend routines to promote independence & participation behavior & mental wellness

Management of Life Plan

Monitoring life plan; communicate and collaborate with members of support team.

Ongoing services provided will include:

  • Weekly meetings with individual & their designated team to monitor quality of services and progress towards goals.
  • Communicate with other professionals and advocate for the individual served including; coaches, support workers, case managers & providers to answer questions, Provide updates & collaborate to problem solve and identify resources.
  • Support the individual & family to navigate & understand the disability and medical system.
  • Explain discovery, relevant case details & offer support to one-on-one support provider or house manager.
  • Update goals & support plan weekly.
  • Provide weekly communication support to the individual as needed for problem solving,
  • advocacy & skill development.  (Time should be identified in the plan, i.e. one hour per week, one hour per day, etc.)
  • Case manager will respond to emails and voicemail within 24 hours Monday through Friday.
  • Crisis response may be provided 9 am to 5 pm Monday through Friday.  In the case of an emergency 911 will be called if needed.

Quarterly & Annual Updates

Reviewing progress and identifying areas for continued growth.

Quarterly & Annual Updates

  • Goals will be reviewed in person and include photos, video and/or written description of progress and/or problem solving & revisions.
  • The case manager will collaborate with the individual and their circle of support to develop an annual update on; quality of life, goals and revisions to supports and services as needed.
  • The annual update is focused on positive outcomes and identifying opportunities to enhance quality of life.
